REGISTRATION FORMS
After acceptance into the Yellow, Groguet, Select, Elite programs, there are 3 forms that each player must mail in to complete the registration process:
1.
USYS for YellowTEAM, Groguet, Select and Elite Players: Medical Release Form
2. US Club Form for 11v11 Elite Players Only: FORM
3. Copy of Player's Birth Certificate All Players: EMAIL
4. Current Player Headshot: All Players EMAIL
Once the forms are complete, e-mail them to [email protected]

Photo, Social Media & Marketing Polity, all members agree to use of photos, videos to use for club marketing use.
Texting, GroupMe and Team Communication Policy, all members will get the club approved communication apps and will use them to communicate with the team. It is the family's responsibility to ensure communications are being received.
